This theme is part of the Simple Desktop Display Manager distribution. This theme is based QtQuick2.
chevron icons are extracted from Font Awesome by Dave Gandy - http://fortawesome.github.com/Font-Awesome.
rectangle.png is from KDM ariya theme.
The wallpaper was taken from here:
